Affidavit of Identity
In order to approve and process your continuing
education credits, Mortgage-Education.com must receive certification
of your identity and an affidavit stating that you will personally
be taking our courses and exams.
Until we receive these items, Mortgage-Education.com
cannot issue a certificate of completion to state agencies on your
behalf.
You may begin (and complete) your studies during
this processing time.
Instructions:
- Print the "Affidavit Of Identity," complete it and
have it notarized. This document must have a notary seal affixed.

- Present a photocopy of your picture identification to the notary
for viewing.
